Payment & Insurance

We are a fee-at-time-of-service clinic, and insurance companies classify our therapists as out-of-network providers. This means that your reimbursement for our services is dependent upon your insurance policies’ out-of-network coverage for outpatient physical therapy.

For example, if your out-of-network coverage is 80:20, after meeting your out-of-network deductible, you may be reimbursed 80% of our fees that your insurance carrier deems reasonable and customary.

As a courtesy and commitment to customer service, our therapists electronically submit your insurance claims for you! Please be sure to bring your insurance card to your first visit if you would like for us to submit your claims.

New patient paperwork; insurance card; any medical records/imaging pertaining to your condition; comfortable, loose clothing (shorts, t-shirt, tank top, walking shoes).
